I, the undersigned, do hereby consent and agree that Canfield Youth Wrestling Club, have the right to take photographs, videotape, or digital recordings of the participant named above during the events and activities organized by the club.

I agree that Canfield Youth Wrestling Club may use such photographs, videotapes, or digital recordings for any lawful purpose, including but not limited to publicity, illustration, advertising, and web content.

I understand that I will receive no compensation if such photographs, videotapes, or digital recordings are used by Canfield Youth Wrestling Club.

I hereby release and hold harmless Canfield Youth Wrestling Club from any reasonable expectation of privacy or confidentiality for the participant named above associated with the images specified.
